CUSTOMER ELECTRONIC CONSENT AND DISCLOSURE
(the "Consent")
Thank you for agreeing to receive documents related to all your AARP Life Insurance Policy(ies) (the “Policy(ies)”) from New York Life Insurance Company (“New York Life”) via electronic delivery. Please review the Consent below. It explains how electronic delivery will work. For example, it addresses:
•   What communications you will receive and over what period
•   How you will receive notification of document delivery
•   What hardware or software you will need to participate in electronic delivery
•   How you may revoke your consent
•   What to do if you fail to receive your communications electronically
Your election to receive documents electronically may not take effect if we do not have a valid U.S. mail address on file for your Policy(ies).
Once you have fully reviewed and agree to the terms of the Consent, please click the “Confirm my settings” button below.
What is the purpose of this Consent?
You have expressed a desire to receive documents related to the Policy(ies) via electronic delivery. To receive documents electronically, you must provide New York Life and its authorized designees with your consent. By electing “On” for Go paperless for all document types available, checking the box stating, “I have read the customer consent & disclosure” and clicking the “Confirm my settings” button below, you will be providing New York Life and its authorized designees, with your consent to the following:
a. receive the Electronic Documents (as defined below) via electronic delivery for the Policy(ies) that you currently own and any Policy(ies) you may obtain in the future; and
b. the terms and conditions set forth below in the Consent.
If you consent, New York Life will transmit the Electronic Documents related to your Policy(ies) by electronic means, to the extent that electronic transmission is consistent with applicable state and federal law. Any document that we send by electronic means, which complies with applicable law, will have the same force and effect as if that document were sent in paper format. New York Life will only transmit documents to you electronically if you consent. Such consent is voluntary. You will be notified by email when your documents are ready to review.
If you do not wish to have the documents delivered to you electronically and/or if you do not agree with all the terms and conditions of this Consent, elect “Off” for Go paperless for all document types available and click the “Confirm my settings” button below or exit the page by clicking “Back to dashboard”. If you do so, we will send you the documents via U.S. Mail.
If your electronic delivery preferences are already on file because you have previously consented to electronic delivery (the “Prior Consent”), unselecting the “Go paperless for all document types available” button will leave your original preferences and the Prior Consent in place. If you wish to withdraw the Consent or the Prior Consent, please follow the instructions below in “What if I change my mind?”
 
What are the Electronic Documents I am consenting to receive? 
You are consenting to receive the following documents via electronic means:
a. General policyowner correspondence including, but not limited to, transaction confirmations; premium notices; notifications of, and deliveries relating to, policy changes and options; claims letters; termination notices; in-force illustrations; or other requests for information or policyowner communications, and
b. any other Documents we may be required to deliver to you in writing.
These documents are collectively defined as the “Electronic Documents.” The Electronic Documents that will be sent to you electronically will depend on the Policy(ies) you own and the availability of these documents for electronic delivery. Even though you have provided New York Life with this Consent to receive the Electronic Documents electronically, New York Life may, at its option, deliver the Electronic Documents to you in paper format by U.S. Mail (for example, if we experience technical difficulties in electronic delivery). In these instances, we will send the Electronic Documents to you at the U.S. Postal address that you have provided to us. For this reason, it is important that you inform us of any changes to your postal address.
 
How will I receive the Electronic Documents?
When new documents are ready for electronic delivery, we will send you an email at the address that you have provided to us. This email will contain a link that will direct you to the AARP Life Insurance Program from New York Life’s Customer Service website https://nylaarp.com where you can view the Electronic Documents. You will be required to login to your online account or create a new account to verify your identity. Once your identity is verified, you will be able to view, download and/or print the available Electronic Documents. Because, in some instances, the date you are considered to have received an Electronic Document from us will determine your rights, please be diligent in ensuring we always have your current email address. Most documents will be available for a period of up to eighteen (18) months from the date on which they are posted, while some may be accessed indefinitely. For those not accessible indefinitely, you may request a copy of the Electronic Document by contacting us at 800-695-5164.
Emails that we send you concerning the availability of Electronic Documents may also contain related inserts, including but not limited to, marketing materials, documentation required to be provided to you in accordance with regulatory rules and other important information regarding your Policy(ies). Documents not made available for electronic delivery will be sent to you through the U.S. Mail.
   
May I obtain paper copies of the Electronic Documents and Consent?
Yes. You may obtain a paper copy of the Electronic Documents and of this Consent by downloading and printing the PDF of the Document. You may also contact New York Life and request a paper copy of the Electronic Documents and/or the Consent at any time and without charge by contacting us at 800-695-5164.
   
Should I maintain copies of the Electronic Documents?
Yes. We encourage you to print or save the Electronic Documents and to keep printed or electronic copies of them for your records and later reference. If you have any trouble with printing or saving, you may contact New York Life and request a paper copy of the documents at any time by contacting us at 800-695-5164.
 
How long will the Consent remain in effect?
The Consent shall become effective once you confirm your settings and shall remain in effect until you withdraw your consent (as described in the next section).
 
What if I change my mind?
This consent is effective until further notice by New York Life or until you withdraw your consent. You may withdraw your consent at any time by returning to the AARP Life Insurance Program from New York Life’s Customer Service website at https://nylaarp.com, clicking on my “My Account” to update your eDelivery preferences in the settings, and electing “Off” for Go paperless for all document types available. Once you confirm those settings, all documents will be delivered via paper by mail. If you withdraw your consent to eDelivery, you understand that you will resume delivery by U.S. Mail for all future Electronic Documents.
 
What if I want to update my Consent?
You can update your electronic delivery preferences (including your email address) at any time by returning to the AARP Life Insurance Program from New York Life’s Customer Service website at https://nylaarp.com, clicking on my “My Account” to update your eDelivery preferences in the settings, electing “On” or “Off” for Go paperless for all document types available, and taking any other required actions.
 
In what form will I receive documents by electronic means?
The Electronic Documents will be provided in Portable Document Format (PDF). You agree that you must have Adobe® Acrobat® viewer software installed on your personal computer or mobile device to view the Electronic Documents. You agree that if you do not have the ability to access and retain PDF documents, you should not agree to the Consent. By agreeing to the Consent, you acknowledge that you can access the Electronic Documents on your personal computer or mobile device in PDF form.  
 
What if my contact information changes?
You must keep New York Life informed of any changes to your email address, cell phone number and all other contact information. You may inform New York Life of any such changes by logging onto the AARP Life Insurance Program from New York Life’s Customer Service Website at https://nylaarp.com or contacting New York Life at 800-695-5164.
 
Are there any hardware or software requirements to do business electronically with New York Life?
Yes. To do business electronically with New York Life and to access and retain the documents sent or made available to you electronically, you must have a personal computer or mobile device with appropriate browser software, and email software as well as communications access to the Internet. Additionally, pop-up blockers must be turned off [in order] to view the Electronic Documents. You must ensure that the mobile device that you are using to review the consent and view the Electronic Documents allows you to access all associated content. This access may incur charges from Internet Service Providers and telephone or cable companies. These costs are your responsibility.
To view the Electronic Documents in PDF, you must have Adobe® Acrobat® viewer software. This software is available for download, free of charge, from the Adobe Web site (www.adobe.com). You must have the ability to save the Electronic Documents to a storage device for later reference or have a connection between your computer or mobile device and a printer so you can print out the Electronic Documents.
 
What if my Email Address is invalid?
If you do not receive an Electronic Document notification because your email address is invalid, New York Life will send you the documents by U.S. Mail using the address on file for your Policy(ies) and automatically remove your consent for Electronic Document delivery should any email notification fail delivery. If you wish to resume electronic delivery of the Electronic Documents, you will need to return to the AARP Life Insurance Program from New York Life’s Customer Service website at https://nylaarp.com, login and click on "My Account", and consent to electronic delivery as outlined above.
 
What if I do not receive documents by electronic means?
You should understand that, from time to time, email may not transmit properly, your Internet Service Provider may experience system failure, the email announcing the availability of your Electronic Documents may be misidentified as SPAM, or other electronic delivery problems may arise. You agree that if you do not receive any of the Electronic Documents by email when expected, you will need to contact New York Life at 800-695-5164. In instances where electronic delivery is not possible, the affected documents will be mailed to you free of charge by U.S. Mail.
If all Policy(ies) that you own with the AARP Life Insurance Program from New York Life are no longer in force and you are no longer granted access by New York Life, you will need to contact New York Life at 800-695-5164 to assist you further.
